This policy applies to puremuze.com (the “Website”). It does not apply to any protected health information, or to any information collected while you are a patient or seeking treatment. Clinical services are delivered by independent licensed clinicians and pharmacies through our HIPAA-compliant infrastructure partner, and protected health information is handled under HIPAA and applicable Business Associate Agreements.
Information we collect
- Personally identifiable information you give us: name, email address, phone number and state.
- Non-identifying information collected automatically: IP address, approximate geographic location, browser and device type.
- Usage details: pages viewed, referring source and interactions with the Website, including responses to the fit check when you choose to submit them.
We collect this information directly from you, automatically as you navigate the Website and in limited cases from third parties such as advertising and analytics partners.

We share information with service providers under contract who perform work on our behalf; with the licensed clinicians and pharmacies delivering your care; with a successor entity in the event of a merger, acquisition, or sale of assets; and where required to comply with legal process, enforce our agreements, or protect the rights and safety of any person. We do not sell your personal information.

State privacy rights
Depending on where you live, you may have the right to access, correct, delete, or obtain a portable copy of your personal information and to opt out of targeted advertising, sale of personal information and certain profiling. Children’s privacy
The Website is not directed to anyone under 18 and we do not knowingly collect personal information from children. If you believe a child has provided us information, email privacy@puremuze.com and we will delete it.

We use administrative, technical and physical safeguards designed to protect your information. No method of transmission over the internet is completely secure and you are responsible for keeping your account credentials confidential.
